January is Social Determinants of Health (SDOH) Month

Many factors impact your health and well-being. Social determinants of health (SDOH) are factors that may negatively or positively impact your health. During January 2025, the Thumb Community Health Partnership (TCHP) will highlight information and resources to help residents address SDOH needs. Follow TCHP this month to learn where and how you can access the services and support.

The Centers for Disease Control and Prevention (CDC) defines SDOH as “conditions in the places where people live, learn, work, and play that affect a wide range of health and quality-of-life risks and outcomes.” Examples of social factors that affect health include:

·        Food security

·        Transportation

·        Job opportunities

·        Health insurance

·        Financial resources

·        Awareness and navigation of services


Individuals including low-income households, veterans, youth, elderly, people with disabilities, and many others, often face barriers to living healthy lives because of unmet SDOH needs. Numerous agencies across the Thumb region provide assistance and resources to those whose health may be impacted by these factors.

What can you do to help?

·        Raise awareness about available services in your community.

·        Reduce the stigma associated with using these resources.

·        Advocate for health fairness in your community.

The Thumb Community Health Partnership (TCHP) serves Huron, Lapeer, Sanilac, and Tuscola Counties and includes 29 health and human service organizations, such as local mental health providers, public health departments, hospitals, community action agencies, and a federally qualified health center. The partnership is focused on improving coordination of services and access to care for residents. Through grant funding, TCHP is creating significant benefits for its members and the broader community.
