CARE Connect Task Force Awards for over $25,000 to Local Organizations

The Thumb Community Health Partnership (TCHP) is thrilled to announce the recipients of the CARE Connect Task Force Mini Projects! With a dedicated $25,014 investment, these grants will empower grassroots organizations to continue their valuable work in communities across Michigan’s Thumb region.
These Mini Projects are designed to support local initiatives that address critical needs, from food insecurity to homelessness prevention, recovery support, and community well-being.
This year’s recipients are:
• Bad Axe Retirees – Supporting community engagement and service among retirees.
• People of Impact – Veteran’s housing support.
• Huron County Homeless Solutions – Emergency housing support.
• WAYGIFT – Substance recovery support.
• Michigan House of Hope – Transportation support for the homeless.
• Huron County Recovery and Prevention Coalition – Transportation support.
• Port Hope Amvets Post 115 – Personal care items and capacity building
.• Family Promise of Tuscola County – Homelessness support.
• Emersion Life Center – Housing improvements.
• Immaculate Conception Catholic Church Food Pantry – Hygiene products.
• Pigeon Ecumenical Food Pantry – Food and hygiene products.
• Millington Ministerial Alliance – Senior education.
• Huron County Baby Pantry – Food and baby supplies.
• Mamas Mission – Food support.
• St. Paul Lutheran Church Caro – Housing support.
• Cross Lex Project Blessing – Food and housing support.
• Good Samaritan Pantry – Transportation support.
Additionally, through a new partnership with Molina Healthcare of Michigan, our collaboration has secured funding to support an additional Mini Project request. Molina Healthcare is providing a $2,500 grant to the Sanilac Food Pantry helping to address food and housing insecurities in the regions served by Molina. This partnership strengthens our collective effort to support those in need and expand vital community services.

The CARE Connect Task Force Mini Projects are a testament to the power of local action and collaboration. By equipping grassroots organizations with resources, TCHP continues to foster a healthier, stronger, and more connected Thumb region.

The Thumb Community Health Partnership (TCHP) encompasses Huron, Lapeer, Sanilac, and Tuscola Counties and includes 29 health and human service organizations including local mental health providers, public health departments, local hospitals, a community action agency, and a federally qualified health center. The partnership is working to improve coordination of services and access to care for residents. The partnership is grant funded and will result in many benefits to members and the community.
Resident-led Mini Projects are supported by the Health Resources and Services Administration (HRSA) of the U.S. Department of Health and Human Services (HHS) as part of a Network Development grant from the Federal Office of Rural Health Policy- An award to the Sanilac County Community Foundation on behalf of a cross-sector partnership totaling $1,200,000 with 0% financed with nongovernmental sources.
