The Thumb Community Health Partnership (TCHP) is celebrating the powerful results of its CARE Connect Task Force’s Social Determinants of Health (SDOH) Mini Project initiative. With $24,984 in mini grants awarded to 17 local non-profit organizations, the lives of residents across Huron, Lapeer, Sanilac, and Tuscola counties are stronger and healthier.
Reaching Thousands of Residents
Through these projects, 3,340 residents were directly supported:
1,250 in Huron County
580 in Lapeer County
1,312 in Sanilac County
198 in Tuscola County
Each project was designed to reduce barriers to health and improve quality of life, meeting people where they live, work, and play.

Meeting Needs That Matter Most
The mini projects focused on the everyday challenges research shows directly impact health and well-being:
28% Food Access – reliable access to nutritious meals
25% Housing – safe and stable living conditions
19% Transportation – access to jobs, appointments, and resources
16% Social Connections – reducing isolation and building community ties
6% Health Services – access to essential care
6% Education & Training – skills for long-term stability

Community-Led Solutions
“These projects demonstrate the power of community-led solutions,” said Kay Balcer, Director of TCHP. “By addressing the everyday barriers people face—like food, housing, and transportation—we’re not just improving health today but creating stronger foundations for healthier futures.”
The Mini Projects, designed by the CARE Connect Task Force, reflect TCHP’s commitment to addressing health beyond the doctor’s office. By investing directly in local nonprofits, TCHP is helping reduce disparities and strengthening the social fabric of Thumb communities.

A Stronger, Healthier Thumb
The outcomes show what’s possible when communities come together to meet real needs. The investment of less than $25,000 is already creating ripples of positive change, from putting healthy meals on the table to ensuring families have safe housing and access to transportation.
