A coordinated regional effort to a healthier community

WHO ARE WE?

The Thumb Community Health Partnership is located in the rural area of Michigan referred to as the Thumb.

The partnership encompasses Huron, Sanilac, Tuscola, and the rural portion of Lapeer Counties.  Members include public mental health authorities, a private mental health agency, public health departments, hospitals, and a human service agency.

MISSION

The Thumb Community Health Partnership will provide an integrated approach to identify key issues and establish a coordinated response to regional community needs.

VISION

To provide a coordinated regional effort to identify, plan for, and provide interventions that will lead to a healthier community

Priorities

Obesity-related chronic diseases and behavioral health are significant issues. Untreated and uncontrolled, these conditions are compounded by limited services and barriers to accessing services. These circumstances interfere with health and wellbeing in our rural community.

Our Story...

Living in a rural community is often perceived as idealistic. Rural Michigan is where many families go to play, relax, and enjoy the beauty of the Great Lakes.

However, the tranquil rural setting in the Thumb area of Michigan can mask significant health problems. Making health disparities worse are poverty, lower education levels, and limited income opportunities.

Beginning in 2018, our multi-sector partnership embraced the opportunity to join our neighbors, family, and friends on the journey to optimal health and a healthier community. We recognize that our collective resources and collaboration are critical to effectively address the persistent barriers and conditions that prevent all individuals from achieving greater levels of health.

The Thumb Community Health Partnership is happy to create a partnership with local health and human service providers to help build a healthier community. TCHP is dedicated to bring the community together to help overcome the barriers that our rural communities face. They want to keep the community informed with resources and service that available right here in our community. For more information about what TCHP is involved in check out the documents below!

 

 Grant Funding

The Thumb Community Health Partnership was formed in 2019, and encompasses Huron, Lapeer, Sanilac, Tuscola. The Thumb Community Health Partnership’s Mission is to provide an integrated approach to identify key issues and establish a coordinated response to regional community needs, thereby providing a cohesive regional effort to identify, plan for, and provide interventions that will lead to a healthier community. 

TCHP members recognize that a unified approach to address mental health and obesity related chronic disease is a priority in the Thumb.

TCHP members include local community mental health agencies, a private mental health agency, public health departments, nine local hospitals, and the Human Development Commission.  These organizations have joined forces to address physical and behavioral health issues in Huron, Lapeer, Sanilac, and Tuscola Counties.
